You may submit this form electronically at the bottom of the page, or print this form and mail the completed form to the Land Survey Program at PO Box 937, Rolla, MO 64502.

Note: Please contact the Land Survey Program's Geodetic Section about a destroyed monument or any monument in danger of being destroyed at 573-368-2300.
